I understand that participation in tackle football and cheerleading involves inherent risks, including but not limited to, serious injury, paralysis, permanent disability, or even death. I knowingly assume all such risks, both known and unknown, and assume full responsibility for my (or my child’s) participation in the Camp.*

In the event of an emergency, I authorize Carlsbad Elite Athletics, its staff, coaches, and medical personnel to administer first aid and/or seek medical treatment for me (or my child). I understand that I am responsible for all medical expenses incurred as a result of participation in the Camp.

I hereby release, waive, discharge, and hold harmless Carlsbad Elite Athletics, its directors, officers, coaches, staff, volunteers, affiliates, sponsors, and representatives from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury sustained during participation in the Camp.

I understand that Carlsbad Elite Athletics does not provide medical or accident insurance for Camp participants. I certify that I have adequate medical insurance or the financial means to cover any injuries sustained.

I agree to abide by all rules and regulations set forth by Carlsbad Elite Athletics and understand that failure to do so may result in removal from the Camp without refund.

I grant Carlsbad Elite Athletics permission to use photographs, videos, or recordings of me (or my child) for promotional or educational purposes, without compensation.

As the parent/legal guardian of the minor participant, I acknowledge and consent to the terms of this waiver on behalf of my child.*
